Hospital-acquired Infections (HAI) in Pennsylvania 2006 - Examples of Pennsylvania Efforts to Reduce Health Care-Associated Infections


The Institute for Healthcare Improvement Campaigns
The Institute for Healthcare Improvement (IHI) announced two national campaigns aimed at reducing patient harm, injury, and death- one is the infection reduction initiative. Of the interventions related to the reduction of infections, sixty-four hospitals are focusing on the reduction and prevention of MRSA, seventy-two hospitals have initiatives on the reduction of CLABS, seventy hospitals on the reduction of VAPs and sixty-nine facilities are concentrating efforts around the reduction of surgical complications including appropriate antibiotic administration.

VHA Hospital ICU Project
Participating hospital ICU teams continue to demonstrate continued improvement in both process and outcome measures for the third consecutive year. Ventilator-associated pneumonia rates have been cut in half since the start of the program. Participating hospital teams also have made tremendous improvements in achieving tighter glycemic control of ICU patients as a means to prevent infection. A total of 27 hospitals are participating in the project in Pennsylvania, New Jersey, and Massachusetts. VHA estimates that these clinical improvements resulted in a projected annual savings of $13 million.

VHA MRSA Project
Beginning in 2006, VHA kicked off a national effort to eliminate MRSA. The program, "Accelerating Improvement Network: Eliminating MRSA," is open to all hospitals, both VHA and non-VHA hospital members.

Quality Insights of Pennsylvania Surgical Care Improvement Project
Quality Insights of Pennsylvania, Pennsylvania's Quality Improvement Organization, worked with a group of Pennsylvania hospitals to improve surgical care, including reducing the incidence of selected surgical site infections. Twenty eight of Pennsylvania hospitals were selected to participate.

The Health Care Improvement Foundation
The Health Care Improvement Foundation's (HCIF's) Partnership for Patient Care is an ongoing, multi-year, regional collaborative committed to improving health care services and enhancing public trust and confidence in the health care delivery system by enhancing patient safety and promoting the use of evidence-based medicine best practices in hospitals throughout Southeastern Pennsylvania. To date they have documented a 21% improvement in antibiotic prophylaxis, a 27% improvement in glucose control, and a 9% improvement in central line insertion and maintenance.

Southwestern Pennsylvania MRSA Prevention Collaborative
The collaborative is a health care community owned and operated model for eliminating hospital acquired health care-associated MRSA infections in acute care and long-term care facilities. Most hospitals participating in the collaborative have elected to use a model based on a guideline which was recommended by the Society for Healthcare Epidemiology of America (SHEA). Following implementation of this strategy in one unit each at the VAPHS and UPMC, MRSA infection rates in both units have dropped by 85 percent over a three-year time period between 2003 and 2006.

The Pittsburgh Regional Health Initiative
The Pittsburgh Regional Health Initiative supported infection control demonstrations in a number of hospital units around the state in 2006. PRHI'S collaborative efforts with the VA Pittsburgh Healthcare System has all but eliminated (methicillin-resistant staphylococcus aureus) infections in one post-surgical unit. The work began as a joint venture in 2002 between the VA, Pittsburgh Regional Health Initiative (PRHI) and the Centers for Disease Control and Prevention (CDC).
